You will be added to the Clinic 38 AVA Connect Portal Labs Requisition and information documents have be released. | Print your lab requisition and complete the bloodwork if not done in the past week. | |
You will be scheduled an Intake Call with the Nurse within 1 business day of your call. | Please accept blocked calls. | |
Once your labs and other testing are complete, you will have a nurse call to provide education on medications. | ||
The physician will call you the same day to send your prescription and review important information. | ||
Follow-up: 48 hours after complete of medications a Symptom Review Questionnaire will be sent to your email. |
